As you might guess, there are multiple surgical techniques, and even some non-surgical ones used to reshape and refine the nose.

Rhinoplasty surgery is usually classified as either “open” or “closed.” These are the techniques surgeons use to access structures under the skin and create the desired contour changes.

The method Dr. Kallman uses for his Miami Rhinoplasty patients is usually “open.” It allows him a high level of precision and visibility for incredibly detailed work.

Let’s take a closer look at why a rhinoplasty surgeon might prefer either option.

Closed Rhinoplasty in Miami

Also called endonasal rhinoplasty, this approach is less invasive but less common too. The surgeon makes small incisions inside the patient’s nostrils, where they are entirely hidden.

With endoscopic tools, visibility inside the nose is possible and tissue can be altered or removed. Once tissue reshaping is done, the incisions are closed with dissolvable sutures. A septoplasty or septum repair can be done via this method, and the procedures may take anywhere between 1-3 hours.

Closed rhinoplasty does take less time in most cases and can mean less bruising or swelling to content with post-operatively. Any scars will be entirely hidden.

However, there are some significant limitations to the closed approach.

When working through extremely tiny openings and using endoscopic cameras and tools, precision adjustments, and complex repairs aren’t possible.

The Open Rhinoplasty Approach

Also known as external rhinoplasty, there are good reasons that 99% of Dr. Kallman’s Miami Rhinoplasty patients have this method. Open rhinoplasty allows clear visualization and access to the entire nasal structure and tissues, including skin, muscle, cartilage, and bone.

The procedure begins with making a small incision under the centre of the nose where it meets the upper lip. This spot is called the columella.

Next, the surgeon lifts nasal skin to reach the nasal framework and make changes underneath. Though it is pulled up during surgery, the skin retracts beautifully, and the incision will become just a small line across the columella. The remaining scar is remarkably faint and usually undetectable under the nose.

Alarplasty (nostril surgery) can also be done simultaneously, and the scars will be situated in the nostril crease, so they blend well once healed also. Open rhinoplasty may take between 2-5 hours in the OR, depending on the specifics of the case.

Recovery After Your Rhinoplasty In Miami

Most people who wonder if rhinoplasty is painful are delighted to find out recovery can be a breeze. Dr. Kallman uses local anesthetic in the nasal tissue during the procedure. That means his patients experience long-lasting numbing effects and comfort after surgery. He also uses atraumatic techniques, so swelling and discomfort are dramatically reduced.

You won’t have nasal packing to contend with after your rhinoplasty, either. Dr. Kallman advises patients to apply icepacks every 20 minutes for the first couple of days once home. Plus, keeping your head elevated will be critical because bending over, lifting, and any strenuous activity can increase blood pressure to the face.
